I, by this release and waiver agreement, give my permission to CHKPix ("Craig Kelley Photography / www.CHKPix.com") to use my (or my child's) digital images for general use on the www.CHKPix.com website and/or for publication in print media. I confirm that, as of this date of signing, I am eighteen years of age or older, and if indicated on the waiver/consent form, acting on behalf of my child or minor of which I have guardianship. I can revoke this permission and release at any time by providing written, confirmed notice to CHKPix.
Since www.CHKPix.com is in the public domain, I understand and agree that digital images found on www.CHKPix.com may be taken, downloaded, and/or released to the public. Therefore, I hereby release and waive claims against CHKPix, its agents, employees, representatives, and any other persons involved with taking or producing the digital content from any and all liability which may or could occur.
I release and forever discharge CHKPix from any and all claims and demands that may arise out of or in connection with the use of the digital images, including without limitation any and all claims for libel or violation of any right of publicity or privacy. I am a legally competent adult and have the right to contract in my own name. I have read this document and fully understand its contents. I have had an opportunity to consult counsel. This release shall be binding upon me and my heirs, successors, legal representatives, and assigns.
Further, I hereby release, discharge, and agree to hold harmless CHKPix from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of such photographs from the www.CHKPix.com website or in any subsequent processing of them, as well as any publication of them, including without limitation any claims for libel or violation of any right of publicity or privacy. I have read the above authorization, release, and Agreement, prior to its execution, and I am fully familiar with the contents of this document. This document shall be binding upon me and my heirs, legal representatives, and assigns.
